All players must return their registration forms to Michael Wilders no later then August 24th.

ANY PLAYER WHO DOES NOT RETURN THE FORM BY THE ABOVE DATE WILL NOT BE REGISTERED IN TIME FOR THE FIRST GAME AND WILL NOT BE AVAILABLE FOR SELECTION.

It is your responsibility to ensure you complete and return the above. If you are not able to return the form to Michael Wilders please contact Mark Moloney who will also accept the form on the clubs behalf.
